Resources I Created During PT School

These are some of the charts that I’ve made during PT school that I think are helpful for anyone to use or fill out. I’ve included blank and filled in versions, but I recommend doing the research and completing them yourself. Please double check with your own notes – my information may be outdated, incorrect, or different from what your professor wants you to know.

The oldest charts are at the bottom of the page, so newer ones will be posted at the top once I upload them. The preview on Google Drive also messes with the formatting sometimes, but downloading the files will make them look exactly how they’re supposed to.

I hope that this doesn’t need to be said, but please don’t claim that anything is your own work. If you have a similar assignment, feel free to borrow some ideas, but cheating isn’t worth your time or your degree.
